About Us
At our clinic, we go out of our way to make your physical rehabilitation and recovery a great experience. You can see us with or without a prescription from your doctor! If you see us first, we can provide you with a complimentary injury screening or refer you to a great physician, if necessary. We know your time is valuable, so to make you wait as little as possible we even offer appointments within 24 hours of your first phone call. We’ll also do everything in our power to accommodate your busy schedule.

I have just completed 6 weeks of physical therapy at this facility following surgery for a partial knee replacement. I can’t say enough about the personalized care and attention I received here. Jonathan and Nancy really listened to my concerns and were there to help me every step of the way. Jonathan also made adjustments in my program during the course of my therapy when necessary to fit my individual goals for a good outcome. I have been to several PT facilities over the past several years and PTSIR Hazel Crest surpassed them by far. Would highly recommend!

Jon is super Awesome. I have been dealing with nerve pain from a car accident for years making it difficult to even touch certain spots of my body. After 3 weeks here I am finally able to touch my legs and not jump out my skin.Amazing how his techniques allow me to live a much normal life and reduce the annoying pain I've had for years.If you need a therapist that understands you, listens to you, and truly cares about your healing and progress come here.
